How to Upload a Request Attachment

This is a how-to guide on uploading a request attachment. A request attachment is typically used for custom consent forms that a provider requires to process your request, and these forms will be emailed to you for completion if needed. Additionally, if you have a specific document that you would like sent with a particular request, you can upload it as a request attachment so it will automatically be forwarded with the request during each follow-up. Please note: this is not the place to upload government photo IDs, SDM documents, or base consents with only the signature, as these uploads will be rejected. 


1. Navigate to the Request Tracker.


In the search bar, enter the Request ID number, client name, or file number to locate the request you want to upload an attachment for.

 

2. Upload your attachment.


Scroll down to the “Request Attachments” section and click the “Submit New Document” button to upload the custom consent form or any additional attachment.


Please note: Be sure to follow the instructions provided in the email when the attachment is sent to you and complete it in full to avoid the attachment being rejected.



3. Confirmation and next steps.


Once your attachment is uploaded, you will receive a confirmation email indicating whether the consent has been approved or rejected. If it is rejected, carefully review the consent form to ensure all required fields are completed, and refer to the instructions in the original email to identify any missing or incorrect information before resubmitting.


Congratulations! You have successfully uploaded a request attachment and now know how to ensure it is completed correctly for approval.
