Why your OHIP request has been rejected - Incorrect Client Address

If you have recently submitted an OHIP request and you have received an email indicating that your request has been returned, it might be due to the following reason:

The client address on the OHIP authorization form does not MATCH what is listed in Service Ontario for the client. 

If the client has recently moved or has moved in the past, their address may not have been updated in Service Ontario's database. 

In accordance with provincial legislation, if you have moved, you need to change the address on your health card, driver’s license, and/or vehicle permit online. When you update a driver’s license or vehicle permit, you must also update the address on your health card so the Ministry of Health has your new address on file.


Option 1: Have your client update their address with Service Ontario (eSignature)

  1. Your client can choose to update their address with Service Ontario using Service Ontario's online portal. 
  2. Once the client confirms they have updated their address with Service Ontario, please update their address in their Medchart profile:



  1. After you have updated their address, you may send the authorization to the client via email to re-sign. This must be done from Action Items by clicking Missing Consent > Request eSignature. 


  1. Once the client completes the form, you will receive an email notification upon which you can Submit the request from Action Items.



Option 2: Have your client update their address with Service Ontario (uploading authorization)

  1. Your client can choose to update their address with Service Ontario using Service Ontario's online portal. 
  2. Once the client confirms they have updated their address with Service Ontario, please update their address in their Medchart profile:



  1. After you have updated their address, the completed OHIP authorization form can be uploaded to the request from Action Items. Section 2 of the form must have the correct address of the client. 
  2. Go to Action Items, click Missing Consent > Upload OHIP Form.
     
  3. Once the form has been uploaded, click Submit.
