How to Create a New Provider for Record Requests

If you are creating a request for a provider whose details are not listed in our database, you have the ability to add them to our database and continue with making your request.



1. After confirming that your provider is not listed in our database, scroll to the bottom of the results table and click Create New Provider.






2. Fill out as much information of the Create New Provider pop-up as you can and then click Create Provider. 


Please Note: We will verify this information to ensure the request is sent to the appropriate party. The more information you are able to provide, the quicker it is to verify with the provider. 

3. You will be prompted to begin the request process for that provider.



Congratulations! You have completed all the steps to add a new provider.
