How to Request Ambulance Call Reports

If you wish to request an Ambulance Call Report for your Client, you will need to direct your request to a Paramedic Service. Requesting directly from the hospital your Client was taken to will likely result in longer than usual turn around times as the majority of hospitals do not store these records themselves.

1. Use the Provider search to look up the Paramedic Service your Client used.

Paramedic Services are regional, so it is important to know what city your Client's incident occurred in. For example, if your Client was involved in an MVA in Brampton, you would need to request from Peel Regional Paramedic Services.

2. Once you have identified the Paramedic Service, click on their entry.

3. You will then be asked to review their details to ensure that the selected Provider is the one you'd like to request records from. To confirmed, click Select. If you need to change any of the provider contact details, select "click here to edit" at the bottom of the pop-up.

4. Select your date range.

5. Enter the Date & Time of Ambulance Call, the Address/Intersection & City of Accident, and the Destination Hospital. This information is required in order to process the request. Click Next when completed.

6. Optional: Add a special note to the provider (ie. a previous or alternate name that the client has gone by or specific event details) and/or mark the request as urgent (urgent service fees will apply - please reach out to our Customer Support Team if you have any questions about this service).

If you do not wish to add anything into these fields, simply click "Next" to proceed.

7. Lastly, you will have the option to Sign & Submit the request (if your client has signed the appropriate authorizations). Otherwise, you can Add to Action Items and continue with submitting the request later on.

Good job! You've learned how to successfully request an ambulance call report.
